Carers Agreement
- Carers and Young Carers are identified when completing new registration packs. They are encouraged to let us know via social media and our website so they can be coded as Carers accordingly. We run regular reports to show all patients who are carers and invite them to groups & services. Centre 33 Young Carers encourage young people they are supporting, to let their surgery know they are a Young Carer.
- As a practice we will be aware of those we think may be carers through our consultations and encourage them to register as one with us, as well as signposting them to support.
- We facilitate a monthly Carers Groups at Rainbow resource centre in Ramsey.
- We have invited all existing carers to our groups and will invite new carers identified.
- Carers are made aware they are entitled to annual health checks if requested and influenza immunisations.
- The Carers Champion will refer into Caring together, encouraging them to create a "What If" plan, access emotional support and register with the service so they can access updates.
- We will promote carers support and national campaigns on our Website and Social Media.
- Care Champion will speak regularly at the Clinical Governance meeting and promote Carer support and awareness.

Carers Groups
We run a monthly roaming carers support group between Manea, Doddington & Ramsey 11:30 to 12:30

Not sure if you’re a carer? A carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction cannot cope without their support.
If you are a carer please let us know at your next appointment so we can check you are coded – coded carers will receive monthly reminder invites to our group.
