- Change of address/contact details - please note, to change your name, proof of ID in person is required at your local branch.
Our team will ensure your query is directed to the most appropriate person to assist you within 5 working days.
This service will help patients that struggle to contact the surgery via telephone during working hours, giving patients the freedom to contact us at their own convenience.
